Two campers from the past return to the game as the Campers are forced to fend for themselves. The campers then face an excruciating challenge with individual immunity at stake. Somebody gets voted off.

The female campers are relaxing by the shore of the camp, though there is clearly a level of distrust between them. Heather is about to snap at Lindsay for a minor mishap when she is interrupted by the sounds of the male campers returning on a yacht from their time at the spa. The guys talk about the luxurious treatment they received, but Owen's offering of a plate of chocolate covered cherry blossoms is rebuffed by the ladies with LeShawna throwing her (right) sandal, then knocking the plate out of Owen's hands and falling into the lake. The guys are okay with this, saying the girls are just jealous, which they are.
Moments later, Chris announces over the PA speakers that as of that moment, the teams are dissolved and every camper is on their own. Chris is not done with the surprises, however, and moments later, Eva returns to the island (with dramatic music playing and a forty-foot wall of fire behind her). Eva declares she's going to pay special attention to the Bass team that voted her off earlier in the competition, though her early elimination was mostly Heather's fault. Gwen openly questions why Eva returned when Chris stressed repeatedly that once a camper is voted off, they can never return. Chris replies that he lied and LeShawna dissuades Gwen from arguing the matter. Bridgette quickly learns that she is the focus of Eva's attention and desire for revenge for remarks made earlier. Chris goes on to welcome the second returning camper: Izzy, who swings in on a jungle vine. Izzy states that she never left the island and was living in the woods while hiding from the RCMP. With that, Chris tells the campers to meet in the amphitheater for their next challenge.
Eva enters the girls' cabin and gets into a confrontation with Bridgette but LeShawna quickly gets between them, to Bridgette's relief, and in a confessional, LeShawna states that Eva is getting on her nerves. Later, at the amphitheater, the campers are seated in a multi-level panel area and Chris says the next challenge will be one of torture called "Say Uncle". Chris says any camper who backs down from a challenge or does not last the minimum ten seconds will be eliminated and the winning camper will receive not just invincibility, a luxurious trailer as well. After introducing a mask wearing Chef as his assistant, Chris spins the Wheel of Misfortune for Duncan's challenge. For the challenge "Turtle Puck Shots", Duncan is forced to stand in front of a hockey net while Chef shoots grumpy, snapping turtles at him. Duncan, while unprotected, gets a turtle biting him in the crotch. Duncan groans and Chris antagonizes him with asking if Duncan is having fun, to which he answers sarcastically.
For Lindsay, her challenge is Marshmallow Waxing, which has Chef applying a melted marshmallow mixture to her face for ten seconds, and she succeeds and moves on. Lindsay, with Eva's "encouragement", chooses Bridgette to be the next participant and her challenge is to stand in a barrel of leeches for ten seconds. Geoff volunteers to do the challenge in place of Bridgette but falls just short of the ten seconds required and is eliminated and takes his place in stocks used to display criminals in the Middle Ages. Owen is next and his challenge is wearing a pair of wooden shorts and enduring a woodpecker. A montage follows showing Gwen getting a nose hair plucked, DJ being enveloped and squeezed by a boa constrictor and Bridgette wearing a shirt composed of live bees.
Chris spins the wheel and says the next challenge will be spending ten seconds inside a large wooden crate with Sasquatchanakwa. Bridgette, with the other campers encouraging her, picks Eva for the challenge. Eva enters the box and emerges after ten seconds wearing a hat and boots that looked to be made from the beast. With Eva's success, Bridgette is eliminated and Chris goes on to show a series of clips that feature how other campers were eliminated and showed less conquer then Eva and cried uncle: Trent failing to jump over a row of skunks sitting between rocks, Gwen not handling ten seconds of New Age music, Lindsay unwilling to have her hair cut by Chef using a chainsaw which Chris calls the "Wawanakwa Hair Salon" and Owen unable to eat a large amount of ice cream without experiencing a brain freeze. Izzy is shocked by a pair of electric eels and likes it so much she asks to have it done again. Chris tells her she can pick the next participant and she chooses herself for a poison ivy spa treatment. Chris says the judges will allow this and she lasts the ten seconds but Chris points out that in doing so, she eliminated herself.
Chris announces that after twenty rounds of tortures, Eva and LeShawna are the final two contestants remaining. Chris tells LeShawna her challenge is participating in a log roll with a grizzly bear named Molotov that Chris points has been the European log rolling champion for the past twelve years. Chris says LeShawna must stay on the log for ten seconds and avoid certain death in the piranha infested waters around it. Chris gives LeShawna the opportunity to back out but she refuses and in the challenge, she stays on and Molotov falls in the water and emerges moments later almost completely stripped of his fur. With that, LeShawna wins individual invincibility and the brand new trailer full of food.
At the campfire ceremony later that night, Chris tells the campers that he's reviewed the confessional vote recordings and he has decided to show what people said and who they voted for. In the recordings, Heather votes for Eva, Duncan votes for Heather, Gwen, Bridgette, DJ, Geoff and Lindsay vote for Eva and Eva votes for Heather. With that, Chris says Eva, with six votes, has been voted off and Eva is shown in a straight jacket on the Boat of Losers moments later screaming that she'll get her revenge and saying that Izzy lied about where she spent her time but Izzy quickly changes the subject before that part continues and at her suggestion, the campers head off to LeShawna's new trailer to party.
Elsewhere, the Sasquatchanakwa and Molotov are in a remote cave and are seen trying on some of the camper's clothes to cover their missing fur. The Sasquatchanakwa puts on a hat that somehow looks like Geoff's lucky hat, and Molotov tries on Sadie's clothes, complete with a Katie/Sadie squeal.

- Challenge 1: Campers were to individually participate in gruesome challenges, or be eliminated. Each camper that passes their challenge, stays in the main game. The winning camper receives a trailer, able to be taken home at the end of the series, and invincibility from the campfire ceremony.
- LeShawna wins over Eva when she stays in the competition, by winning the final challenge against Molotov the Bear.

Trent: I'll never forgive myself for that one. You know, you're totally the last person here I'd leave buried in the sand if I had a choice.
Gwen: Really? That's so sweet.

Lindsay: Awww, that is so romantic!
- Eva: (to Bridgette) Here's a toe tag, surfer girl. You'll be needing it for later.
- Chris: Izzy, who's your next victim?
Izzy: Ooh, ooh, me, with a poison ivy spa treatment.
- LeShawna: (about Izzy) Is it just me, or is that girl some kinda crazy?
- Chris: (after phone call) Okay, the judges will allow it, but they want to know why.
Izzy: I just wanna see how it feels.
- Chris: LeShawna, it's up to you. Your final challenge is the Grizzly Bear Log Roll.
LeShawna: The Grizzly Bear say what?
- LeShawna: (about the bear) I've seen scarier looking faces at the mall.
- (After LeShawna beats the grizzly bear)
Chris: LeShawna wins, so Eva is out.
Eva: WHAT? No way!
Chris: Waaaaaaay. She wins the challenge, invincibility and the grand prize.
- LeShawna: (to Eva) You lose, I win, you lose, I win...
- Chris: Okay, so first off, we ran out of marshmallows.
Owen: NOOOO!

Duncan: I vote for Heather, because I know she's behind Courtney getting kicked off. You'll pay for that, Toots. (to Courtney) If you're watching this on cable, I miss you, Babe.
Gwen: Eva's a freak. So see ya.
Bridgette: Please, please, Eva. I'm so glad you never air these.
DJ: Eva's nuts. Sorry, girl.
Geoff: It's gotta be Eva...unless I can figure out who snagged my other lucky hat.
Lindsay: (rubbing her skin) I just can't get over how smooth this is. Anyway, I vote off Ava, cause she's scarier than Heather, Laquesha, and Gwen combined.
Eva: (Only her menacing eye is showing) Unless they want to leave in body bags, they better not say my name. I vote for Heather.
- Chris: Lots of dirt revealed there, huh? But, in the end, it was still six votes against Eva. So, adios!
Eva: What?!?! This isn't the end of me! You better watch your backs!! I'm not done! I'll get my revenge! (on the Boat of Losers in a straight jacket) I'm not done with you!!! Oh! and Izzy lied. She wasn't in the woods, she was...
Izzy: Wow, she has issues, huh? Party at LeShawna's new crib!

- Title Reference: This episode's title and content parodies the known phrase, No Pain, No Gain.

- Izzy and Eva returned to camp.
- Duncan and Eva are the only ones shown to vote for Heather.
- The two returning campers were both a female from the Killer Bass and a female from the Screaming Gophers.
- This is the first time Izzy yelled like Tarzan.
- Even though it was mentioned in the recap of the next episode, this episode's title is the first time Chris has ever mentioned during his regular speaking.
- It's shown in this episode that the campers vote each other off by saying who they want off in the confession stall, however in Basic Straining, it's shown that they vote each other off using a ballot box. Though this was probably done to prevent what Harold did in Basic Straining. This is also done in Total Drama Action.
- It's a mystery as to why Duncan would think Heather had something to do with Courtney's elimination, as she was on the opposing team and therefore couldn't have voted against her. Though he may have thought she may have influenced someone to vote her off due to past influences.
- LeShawna, Trent, Izzy, and Owen are not shown to have voted anyone off in this episode.
- When Izzy returns, she is eating a raw fish like a wild animal. However, in The Sucky Outdoors she bit a raw fish and then stopped, saying that they should cook it first. She may have just said that because the other campers found it weird for her to eat raw fish.
- The reasons Izzy and (especially) Eva returned are a mystery, though it's possible that Izzy may have been allowed to return since she was not officially voted off before and Eva may have been brought back because Chris knew she would cause drama for the campers, which would bring in higher ratings combined with Izzy's crazy personality.
- Izzy tells everyone that she hid in the woods when the RCMP came for her and while there, she befriended a family of beavers. Eva claims that Izzy was at Playa Des Losers, however, but Izzy interrupted her before Eva could mention the location.
- At the end of the episode, the Sasquatchnakwa and bear that were mauled during the challenge can be seen wearing some of the camper's clothes. The Sasquatchnakwa is wearing Geoff's hat and the bear is wearing the clothes that were worn by both Katie and Sadie (most likely Sadie's clothes due to the size). At the end of Who Can You Trust Sadie appears without her luggage that appeared in Not So Happy Campers - Part 1, the Bear may have taken it.
- Owen, Izzy, Lindsay and Gwen are the only ones who did more than one challenge.
- Here are the challenges the campers had to go through:
- Owen: First he had to wear a pair of wooden pants with a woodpecker attacking him. Then he had to eat a dozen gallons of ice cream and withstand brain freeze.
- Geoff: He had to sit in a barrel full of leeches.
- Bridgette: She had to be covered with live bees.
- DJ: He had a huge snake wrapped around him.
- Duncan: He had to have snapping turtles thrown at him while he was unprotected.
- LeShawna: She had to log roll against a bear above piranha-infested water.
- Izzy: First she had to be zapped by two electric eels. Then she had her head wrapped in poison ivy.
- Lindsay: First she had to have hot melted marshmallows poured on her face. Then she had to get a hair cut with a chainsaw (she escaped before Chef could reach her hair).
- Eva: She had to go into a cage with Sasquatchanakwa for ten seconds.
- Trent: He had to jump rocks while dodging a bunch of skunks.
- Gwen: First she had to have her nose hairs yanked out. Then she had to listen to New Age music.
- Heather is the only one who is not seen doing a challenge.
- LeShawna is the winner of the first individual challenge. This makes her the first camper to win solo-invincibility.
- Althought they don´t interact in this episode, Owen can be seen smiling at Izzy as the other characters were glaring at her at the end of the episode.
- The mask chef wears while yanking out Gwen's nostril hairs is the same one in Hook, line and screamer .

- Lindsay's Name Goof(s): She calls LeShawna "Lequesha" again, she mispronounces Eva's name as "Ava", and called Chris "Chip".
- Continuity Error: Gwen is apparently "still" mad at Trent for not digging her up in Phobia Factor, however in past episodes (namely Paintball Deer Hunter, If You Can't Take The Heat..., Basic Straining and X-Treme Torture) she seemed to be perfectly fine working beside him with no signs of a grudge. She even insinuated in X-Treme Torture that the two of them were a couple.
- During the campfire ceremony, Izzy, Owen, Trent, and LeShawna's thoughts on who should be voted off were not shown. In fact, Trent wasn't seen at all in the campfire ceremony.
- Eva tells Bridgette that she recalls her telling her that she needs anger management classes but in The Big Sleep, it was Courtney who said it, in fact, Bridgette didn't even have any lines in that episode.
- In the final round, Chris said that Eva was up for the next challenge, but after he spins the wheel, LeShawna had to do the challenge, which was the log roll.
- The bear is shown wearing Sadie's clothes even though Sadie was voted off a few episodes ago and was at Playa Des Losers at the time of this episode, but the bear might have somehow taken some of Sadie's luggage before she was voted off and saved it.
- When Eva was at her seat, her normal outfit was on, but later when LeShawna won the challenge, her swimsuit was on.
- Izzy wasn't seen with the other campers (Owen, Gwen, Heather, Duncan, Geoff, DJ, Lindsay, Bridgette, Trent, Eva) that were locked up in stocks watching LeShawna's final challenge (the Log Roll).
- It is not explained how the campers managed to go to the dock while still bound by the stocks.
- Chris told Lindsay that she would have her entire body covered with melted marshmallows, but Chef only covered her face. Plus, if her whole face was waxed, that means her eyebrows would be removed, but they remained when Lindsay was done with the waxing.
- At the campfire ceremony, Trent is not present with the others. The campfire normally only accommodates eleven people (As the two teams each had eleven members), so Trent was by some mystery excluded from the final scene with the campers altogether. However, he may have been sitting somewhere far away from the campfire because the skunk sprayed him during the skunk jump, similar to Geoff in X-Treme Torture.
